The aim of this service is to enable people to live well with Dementia by: Enabling easy access to timely and appropriate information and advice throughout the Dementia pathway. Facilitating a range of education sessions and peer support for people living with Dementia and their carer(s) in all areas of Derbyshire. Core contract value £1,850,000 over the full term of the contract including extensions. The additional £500,000 is optional extension to spend only where need and funding is identified and is not guaranteed.
