About this contract
v. The Council would like to commission a community-based worker/s with a focus on reducing infant mortality, where that individual can use their local knowledge and relationships to have conversations about infant mortality risk factors that provide information or capture information, raise awareness, educate or result in a specific action/activity. It is hoped that by working together Health Services, the Council and Communities can help improve outcomes and reverse the increase in rates. Whilst it is important to address all related risk factors there will a particular need to focus on: - Reducing smoking in pregnancy/smokefree environments - Increasing uptake of genetics advice for consanguineous couples by supporting referrals into the preconception genetic literacy clinic (see appendix) - Safer sleep advice - Other topics to be agreed with the provider based on engagement with communities
