About this contract

The Home from Hospital service is to support the prevention of unnecessary acute hospital admissions, support timely discharge from hospital, prevent hospital re-admissions and maximise independent living. The contract will run for a period of 3 years with the option to extend the contract by up to 2 x 12 month periods at the sole discretion of the council.
