About this contract
Text to be corrected in the original notice: Place of text to be modified: II.1.4 Instead of: Waltham Forest CCG is seeking to commission a primary care-led service model for the monitoring and management of anticoagulation patients. The service is required to be delivered from a minimum of five sites across the borough, with at least three of these sites being local pharmacies. Initially the service will be responsible for the management of approximately 1200 stable patients on warfarin; the CCG will seek to work jointly with the provider and secondary care to further develop the anticoagulation pathway over the life of the contract, which may include extending the service to the monitoring of other types of anticoagulants. The service includes the provision of Point of Care Testing, a domiciliary service for patients who are housebound, and the use of INRstar software for dosing advice and data reporting. Initial 3 year contract from 1 January 2018 - 31 December 2020, with the option to extend for up to a further two years.
