About this contract
This is a pilot scheme to deliver information and training to the citizens of Leeds via a structured training program.Type 2 Diabetes Supported Structured Self-Management Programme for South Asian Communities is a 12 x 8 week structured education programmes for people from South Asian communities delivered to cover 3 areas of Leeds (Harehills/Chapeltown, Beeston and Gipton/Oakwood). These chosen areas are based on the highest levels of South Asian populations matched to an increased prevalence of diabetes This is a contract in 3 lots with a total cost of 40,000 per annum. Lot 1 - Harehills/Chapeltown - 14,000 Lot 2 Beeston& amp; nbsp; - 13,000 Lot 3 - Gipton/Oakwood - 13,000 The total length of the contract including design and the post contract evaluation will be 12 months. Organisations are encouraged to apply for one, all or any combination of the lots available. Please refer to Part 2 Specification document for full service requirements.
