About this contract
NHS Heywood, Middleton and Rochdale Clinical Commissioning Group (CCG) are looking for suitably qualified Providers to submit tenders for The Provision of a Cardiac Rehabilitation Service. The Provision of a Cardiac Rehabilitation Service purpose is to provide all patients who are 16 years and above who fulfill the referral criteria with a patient centred service which optimises their health and well-being, enhances quality of life and minimises the risk of recurrent cardiac events. The objectives for this Service are detailed in the Service Specification. The contract duration will be for 3 years with a Service Commencement date of 1st April 2017. The indicative annual value is £225,500. The contract will utilise a Local Full tariff payment adopted from the National Mandatory tariff and will be that of a stepped approach as detailed with the Tender Pack. The Services to which this Procurement relates fall within Schedule 3 of the Public Contracts Regulations 2015 and the tender process will be conducted as a single stage open process.
