About this contract

NHS Eastern Cheshire are seeking care home Providers, registered for nursing care that will work with the CCG and the wider health and social care team to provide beds for the SUPPORT TO ASSESS SERVICE. The Support to Assess Service described here is for a bed based service which provides support for people who require further assessment of need and care provision. It will provide services both as an alternative to hospital admission and for those patients who have been in hospital and who are medically optimised (no longer require acute hospital care) and waiting further assessment and care provision
