About this contract
Medway Council are seeking an organisation to deliver the Connected Communities Project on the Hoo Peninsula. The Provider will deliver the intervention as set out by the Commissioner for the programme. They will be required to recruit a part-time SPLW to deliver the social prescribing intervention to socially isolated older people with an ME3 postcode. Referrals into the service will be from a wide range of partners and organisations. As well as referrals from the Provider's existing pathways, the commissioner will direct any referrals from the Connected Communities programme, where the referee has an ME3 postcode, to the Provider. The SPLW will contact referrals within 5 working days of receiving them and offer the service user an initial appointment. This should be delivered face to face, either at the service user's home or a location that suits them.
