About this contract

London Clinical Commissioning Group (CCG) commissioners seek applications for the provision of continuing healthcare services in nursing homes. The Any Qualified Provider (AQP) process covers service users eligible for continuing healthcare mental health adult (including dementia) and continuing healthcare physical disabilities adult (including frail elderly) including end of life care. Service users must be over 18 years old. The pan-London CHC in nursing homes AQP is open for applications from 13 November to 22 December 2017. Successful providers will be added to an AQP list which will be used by London Clinical Commissioning Groups (CCGs) and discharge teams to make continuing healthcare placements in nursing homes. The AQP is the standard route of commissioning for the participating CCGs. This means that care packages will be commissioned with AQP providers. Only in exceptional circumstances will packages be commissioned with non-AQP providers.
