About this contract
This tender relates to the previous PIN for Integrated Information Technology System to support NHS Health Check Programme in Nottinghamshire - 9R2K - CKH202 Lot 1 - GP and Outreach Information Technology (IT Solution) 1. The Authority requires an NHS Health Check IT Solution to provide a tool for all GP practices across Nottinghamshire, compatible with GP clinical systems i.e. SystmOne, EMIS LV, EMIS Web, EMIS PCS, plus others not listed here over N3, which will identify the eligible population and monitor individual's status to NHS Health Check completion and a reporting mechanism to provide anonymised reports to aid performance management and national reporting requirements and compliance. 2.In addition, the IT Solution will enable the Outreach Service Provider to deliver an Outreach Service to target hard to reach populations in the community. The IT Solution must be able to identify Outreach NHS Health Check data as activity outside of the GP practice and to feed data through to GP clinical systems i.e. SystmOne, EMIS LV, EMIS Web, EMIS PCS, plus others not listed here over N3. Lot 2 - Outreach Service Nottinghamshire County Council requires a provider to deliver a Targeted Outreach Service. This Targeted Outreach Service will supplement the existing Core Service by targeting Cardiovascular Disease (CVD) priority groups around the county. The Provider will not be expected to deliver the call and recall service but must nevertheless be able to exclude people who have had a check within the 5 year call and recall cycle. The aims of the NHS Health Check programme are to: a) Reduce morbidity, mortality and hospital admissions due to CVD and, b) Reduce health inequalities, including socio-economic, ethnic and gender inequalities, through delivery of a cardiovascular disease risk assessment and management programme.
