About this contract
The Place Based Care Network (PBCN) is a practical tools based learning network aimed at clinicians and senior leaders. It is an enabler in moving Sustainability & Transformation Partnerships and New Care Model (NCM) areas from the 'what' to the 'how' of place based accountable care. It was established in 2016 following consultation with NCM vanguards which identified a specific need for support to aid development, testing and scaling up of new care models and the establishment of a learning network which would address these points. This piece of work is for the further development of the PBCN, aligned with and supporting the Sustainability & Transformation Partnership offer to extend the reach of the network and build on the opportunities as identified by the first pilot cohort. The aims will be achieved through the development of a programme involving successful networking and use of tools collecting data and analytics on various aspects such as change readiness and building capability for transformation in teams. This will be done by a physical network of regional and national events, as well as the establishment of a virtual network (utilising existing infrastructure) to encourage further discussion and as an information repository.
