About this contract
Bromley CCG are looking to tender a service in the next few months to support patients in the community with their medicines, and wish to engage with potential bidders to explore how the service may be developed and delivered. The contract is expected to run for a period of no longer than 2 years. The service will build on the existing Medicines and Assessment Support Service (MASS), working with the CCG to scope and inform how the service should be commissioned going forward. The service provider will be expected to have the infrastructure and the staff with the appropriate level of clinical qualification and experience to enable an effective and efficient mobilisation. The service will be provided at individual patient level and should include, but not be limited to: -Clinical and pharmaceutical assessment of patient referrals into the service -Assessment of medicines adherence -Clinical medication review -Recommendation and support for implementation of personalised tailored interventions to support patients with their medicines -Strong collaborative working with other health and social care professionals
