About this contract

The Kent and Medway STP intend to commission, through Diabetes Transformation funding, a Kent and Medway Diabetes Education Booking Hub. This will enable an easily accessible and flexible route for referral for people from across Kent and Medway with Type 1 and Type 2 diabetes to book places on Structured Education courses. It will include assessment/triage, signposting, booking and include a follow up process for those who don't immediately book.
