About this contract
Buckinghamshire County Council is committed to improving the health and wellbeing of its population and targeting services to those areas of greatest need. As part of a programme of initiatives to improve the health of the most disadvantaged, the Council are seeking a suitably qualified and experienced organisation to provide a Health and Wellbeing Community Development and Health Improvement Programme within Aylesbury Town, Buckinghamshire. The organisation will take a community development approach to health improvement, and through this will build the knowledge skills and confidence of individuals and also build capacity to support health and wellbeing in the local community. The programme will include a Skilled for Health Programme and a programme of health improvement activities. The approach to developing and delivering these programmes will be underpinned by community development approaches and focussed on recruiting target individuals and seldom heard groups into health improvement programmes. The provider must build strong partnering relationships with complementary organisations including local health services, third sector, public and civic organisations. The main components of the health and wellbeing community development and health improvement programme are: ? Community development and engagement ? A general Skilled for Health, (SfH) and Maternity Skilled for Health Programme, both aimed at minority ethnic groups ? A programme of health improvement activities. The Programme of health improvement projects will be developed over year one, creating a baseline of activities and testing uptake that will inform a detailed work programme and targets for years two and three. This will be agreed with the Council and reviewed as a minimum on a quarterly basis. The projects will reflect the needs and priorities related to the target populations including: ? Develop and implement strategies to maximise the recruitment of the target groups into the programmes ? Offer a programme of activities which support local needs and Council health and wellbeing priorities ? To undertake engagement work with the target communities to both identify health needs and user satisfaction of the services available ? Use a variety of community engagement approaches to build individual and community capacity. The contract will be awarded initially for 3 years with a possibility to extend for a further two years, depending on performance and availability of funding.
