City of York Council and the NHS Humber & North Yorkshire Integrated Care Board (the "Commissioners") are looking to commission dedicated Discharge to Assess ("D2A") unit with up to 10 beds on a block purchase basis to support our patients and customers following discharge from hospital on discharge Pathway 2.
