About this contract
The LWC programme, which started in 2016, sets out to transform the way services for people living with cancer are commissioned and delivered across Lincolnshire. The programme is led by Lincolnshire West Clinical Commissioning Group (CCG), as lead commissioner for cancer on behalf of all 4 CCGs in Lincolnshire. The programme is part of the system response to current and emerging national, regional and local policy for cancer services, including: • NHS 10 Year Long Term Plan • 'Achieving world class cancer outcomes: A Strategy for England 2015 - 2020 • NHS England Living with Cancer Programme • Lincolnshire Sustainability and Transformation Plan • Living with cancer Strategy for Lincolnshire 2017 - 2019, Living with Cancer Strategy for Lincolnshire 2019 - 2021 The programme has system wide scope, particularly the transition from treatment phase into 'living with cancer' (known previously as survivorship), which includes out of hospital and community based support, e.g. through local Integrated Neighbourhood Teams (multi-disciplinary teams representing commissioners, providers, local authorities, the voluntary sector and patients). Initially the programme is focussing on nationally defined priority tumour sites, i.e. Breast, Prostate, with future roll out across all clinically appropriate tumour sites.
