About this contract

NHS Wirral CCG is seeking expressions of interest to provide a domiciliary care service to Wirral residents over the age of 18 years, with complex needs who require end of life care. This includes those who are eligible for fully funded NHS Continuing Health Care (CHC), or are in receipt of a joint funded package of care and have complex needs. The service must be able to receive and response to referrals 7 days a week during the hours of 7:30am and 10:30pm. The service is to be provided within a person's own home. The majority of activity supports CHC fast track patients discharged from hospital in the last phase of life.
