About this contract
Derbyshire County Council is seeking a provider for the design and delivery of a self-management programme for people living with long term conditions ('Programme'). The intervention will be designed to empower people that require support to better manage their long term condition (LTC) through a self-management model. The Programme must be available to people within the geographical area of the Derbyshire ('County'), with the exception of Derby City. In line with the national 'Talking Health' approach the Programme will target people living with LTCs; with face-to-face course sessions being delivered by trained volunteer tutor peers (themselves living with LTCs). The aim is to empower and support people by increasing their confidence, improving their quality of life, helping them to manage their condition more effectively and become an 'expert patient'. An expert patient is considered to be someone who: • Feels confident and in control of their life • Aims to manage their condition and its treatment in conjunction with healthcare professionals • Communicates effectively with professionals and is willing to share responsibility for treatment • Is realistic about how their condition affects them and their family • Uses their skills and knowledge to lead a full life The initial contract period is for 2 years commencing on 01 December 2018 to 30 November 2020, with the option to extend for a further two 12 month periods.
