Hywel Dda University Health Board (HDUHB) have an opportunity to expand their provisions of Bereavement Services for the North and South Ceredigion Cluster Areas and are looking to commission a suitable provider to undertake this work for the County’s population.
The contract will run for an initial two (2) year period, and have an option to extend for a further twelve (12) months.

The successful provider will need to be able to communicate and liaise with our population through the medium of Welsh and provide in-person home contacts to the hard-to-reach areas across the rural geography due to limited public transport options, and patients’ inability to travel independently, therefore, this would be a critical factor for the service provision.
The service will deliver a bereavement and anticipatory grief service to adults aged over 18 years to support a patients bereavement recovery and adjust to life after loss via various mediums including but not limited to, telephone, virtually via a suitable online platform and in person.Value not published
