About this contract

Torbay and South Devon NHS Foundation Trust provides and commissions a range of services to support Carers and reduce health inequalities across Torbay. Part of this service is delivered through an 'enabling service' which supports Carers to identify their strengths and needs, supporting them to develop a personalised plan for breaks and support, exploring options and choices for meeting those needs and maintaining their health and wellbeing. This involves delegated responsibilities under the Care Act 2014 for undertaking Carers Assessments, in the form of Carers Health and Wellbeing Checks, and for providing an advocacy service for Carers.
