About this contract
North Staffordshire and Stoke-on-Trent CCGs wish to commission an enhanced primary care service for nursing home residents within the North Staffs and Stoke-on-Trent CCG boundaries. The enhanced service aims to improve the quality of care for residents through proactive care planning and more coordinated management of residents’ complex needs. Residents will have access to timely medical advice and review so that they can continue to reside in their care home for as long as possible, and practices will assist in the development of care staff to recognise earlier signs of ill health. The CCGs are inviting Providers who are able to provide core GMS and enhanced medical support to care home residents and whilst it is expected that this will only apply to local GP practices we are happy for wider collaboration to take place between GPs who may wish to establish a lead practice arrangement. Inline with similar successful projects elsewhere in England, the CCGs aim is to align 1 practice to a care home, as this will ensure continuity for the residents and will allow close working relationships between primary care staff and the care home to develop. For those homes with large numbers of beds or defined units, it is expected that more than 1 practice will may provide enhanced services to the home.
